Menopause Symptom Score

The Menopause Rating Scale (MRS) is the 11-item questionnaire clinicians actually use. Rate each symptom for the past month; you get the validated total, the three domain scores, and a summary you can paste straight into a patient portal.

How severe has each been, roughly, over the past month?

Common questions

What is the Menopause Rating Scale?

The MRS is a validated 11-item questionnaire used worldwide in research and clinics to measure how much menopause symptoms affect daily life. Each item is rated 0 (absent) to 4 (very severe); the total runs 0–44 across three domains: somatic, psychological and urogenital.

What score counts as severe?

The usual bands: 0–4 none or little, 5–8 mild, 9–15 moderate, 16 and up severe. Bands vary slightly between sources — the trend over time, and how you function, matter as much as the number.

Can a score tell me if I'm in perimenopause?

Not by itself. The MRS measures burden, not stage. Pair it with the cycle-rule stage calculator — clinicians look at both.

Method

Items and domains follow the standard MRS (Heinemann et al., validation published 2003/2004; subscales: somatic — flushes, heart, sleep, joints; psychological — mood, irritability, anxiety, exhaustion; urogenital — sexual, bladder, dryness). It's a screening and tracking scale: not a diagnosis, and not a measure of whether treatment is right for you. Repeat it monthly — the slope is the useful number.
